Release of Records: If applicable, you may be asked to complete and sign a Request for Release of Records form that will be sent to your student’s former school.
Please use this checklist as a guide to ensure your registration is accurate and complete.

After completing, a member of our enrollment team will contact you via email about your next steps. - Required documentation, as part of the enrollment process, includes clear, legible copies of the following:
- Original Birth Certificate (click here for information on how to obtain a copy)
- Proof of Residency
- Acceptable Documentation: Utility Bill, Water Bill, Deed, Mortgage Statement, Closing Paperwork, Purchase Agreement or Lease Agreement
- Immunization Record
- Custodial Paperwork (if applicable)
- IEP or ETR (if applicable)
- All documentation can be submitted directly through the PowerSchool Parent Portal, emailed to [email protected], or dropped off at our Central Office located at 2 Warrior Way, Cincinnati, OH 45227.
- Please remember that registration is not finalized until the online registration forms are completed in the PowerSchool Parent Portal and the appropriate documentation is turned in.
